Coaching Commitment Form

BEAUTIFUL OUTCOME

I am excited that you are choosing to do coaching and work through Marriage Thrive.

My commitment as your coach is to show up fully and maximize these calls to help you get the results you want to get.

There are a few important commitments required for you to get the results promised through coaching.

Please read the following commitments:

• I will complete all coursework as assigned.

• I will submit the Intake Form 24 hours before each call

• I will be on time for my appointments.

• If I need to reschedule a coaching call, I will do so as soon as possible. Rescheduling an appointment less than 24-hours prior will result in the call being lost or a $350 rescheduling fee.

Please Note:

Completing homework and submitting the Intake Form are required for Coaching Calls to move forward.

If your spouse will be doing the calls with you, it is required that you both complete this commitment form. We cannot move forward with scheduling your first call until we receive BOTH of your 'signed' forms.
